ENROLLMENT APPLICATION

Children entering Trinity Lutheran School must be the appropriate age prior to September 29th, and toilet trained.

 

Trinity Lutheran School admits students of any race, color, religion, national and ethnic origin to all rights, privileges, programs and activities generally accorded or available to students at our school. We do not discriminate on the basis of race, color, national, or ethnic origin in administration of our educational policies, admission policies and other school-administered programs.

 

Parents who seek enrollment for their children are asked to complete an Enrollment Application for admission, available in the school office or by clicking the enrollment application button above. A non-refundable registration fee must accompany each application. Enrollment applications for the next academic year are provided to families currently enrolled in January of each year. Open enrollment to the general public begins in February. Applications must be completed and returned to school with the appropriate registration fee. 

 

Parent(s)/guardian(s) (hereinafter referred to as "parents") whose children have attended another school or other educational program are asked to bring the most recent report card or similar progress report to Trinity upon application for admission. Trinity Lutheran School will request past school records from the previous school attended; the confidentiality of these records will be maintained. Students without previous academic records will be tested prior to enrollment.  Parental consent to release school records is no longer required by law (Family Education Rights and Privacy Act (FERPA) (20 U.S.C. 1232g; 34 CFR Part 99)).

Parents of each student enrolling for the first time shall show a copy of the State Certified Birth Certificate for student to be enrolled, of which witnessing will be documented by an office employee of the school. A current State of Virginia Immunization Record and health physical must also be provided prior to a child's attendance. The Commonwealth of Virginia School Entrance Health Form (MCH-213E, Parts 1-3 REV. 10/03) is the approved form and can be obtained from your physician or the Department of Health. If you are not the birth parent, legal documentation as to your relationship with the child is required.

 

Upon receipt of the Enrollment Application the school will draw up an Enrollment Agreement (contract) for your child. Please sign and return this contract by the required date.  A child will be officially enrolled when all admission steps are completed.
